The U.S. Food and Drug Administration (FDA) has approved the premarket approval (PMA) application for the Allurion Gastric Balloon System, potentially expanding treatment options for adults with obesity who are seeking a nonsurgical approach to weight loss.

Developed by Allurion Technologies, the system features the Allurion Smart Capsule, a swallowable gastric balloon designed to help patients feel full sooner and eat less.

“Today’s approval is a watershed moment for Allurion and for obesity care in the United States,” said Shantanu Gaur, M.D., Allurion founder and chief executive officer. “The Allurion Smart Capsule offers a safe and effective alternative to GLP-1s and bariatric surgery, and it should become a standard tool in the comprehensive treatment of obesity in the United States.”

How It Works

The capsule is swallowed during a brief, approximately 15-minute office visit. No surgery, endoscopy or anesthesia is required. Once in the stomach, the balloon is filled with fluid, where it takes up space and promotes a feeling of fullness.

After about four months, a built-in release valve allows the balloon to deflate and pass naturally through the digestive system. If additional treatment is needed, a second balloon can be placed about two months later.

Each capsule provides roughly four months of therapy from a single placement, offering a time-limited alternative to daily pills or weekly injections.

Who Is Eligible?

The device is approved for adults ages 22 to 65 with a body mass index (BMI) between 30 and 40 who have had at least one unsuccessful weight loss attempt.

More than 100 million Americans have obesity, and approximately 80 million fall within this BMI range. The system is intended to be used alongside a moderate-intensity lifestyle program that includes nutrition, physical activity and behavioral support.

How It Compares With GLP-1 Medications

GLP-1 medications have reshaped obesity care in recent years, but many patients discontinue treatment within the first year, often due to side effects or cost. Weight regain after stopping these medications is common.

The Allurion Smart Capsule offers a different approach:

  • It is not a medication.
  • It does not require ongoing dosing.
  • It remains in the body temporarily.
  • It does not involve injections.

Clinical data suggest it may also be used in combination with low-dose GLP-1 medications under medical supervision.

What the Research Shows

FDA approval was supported by results from the AUDACITY pivotal trial. Outside the United States, the device has been used in more than 200,000 patients and studied in numerous peer-reviewed publications.

Real-world data outside the U.S. show patients achieved about 14% total body weight loss with one treatment cycle and more than 20% with two cycles when combined with digital support and physician monitoring.

What Patients Should Know

The Allurion Gastric Balloon System is not a permanent solution. Because the balloon remains in place for a limited time, long-term success depends on lifestyle changes developed during treatment.

For adults who are not candidates for surgery or who are looking for an alternative to long-term medication, this PMA approval may lead to another option within the evolving landscape of obesity care.

“Allurion’s innovative solution is an important tool within the continuum of obesity care,” concluded Jaime Ponce, M.D., president and medical director of the Weight Loss Center of Chattanooga and former president of the American Society for Metabolic and Bariatric Surgery. “For patients with a BMI between 30 and 40 seeking meaningful health improvements, the Allurion Smart Capsule could be an attractive option, especially for those seeking a structured alternative to medications or surgery.”
