If you’re taking GLP-1 medications (like Ozempic, Wegovy or Mounjaro), you’ve likely seen impressive weight loss. But maintaining that weight — and protecting your health in the long term — requires more than the medication alone. According to Reema Dbouk, M.D., assistant professor and physician at Emory Healthcare, physical activity is key.
“Exercise is essential for weight maintenance,” says Dbouk. While GLP-1s primarily drive weight loss through appetite suppression and metabolic changes, movement helps patients keep the weight off, especially once the body starts adjusting to the medication.
Aim for Movement That Builds Muscle
Dbouk emphasizes that with rapid weight loss, patients don’t just lose fat — they also lose lean muscle mass. To help preserve strength and metabolism, she recommends resistance training two to three times per week, for at least 20 minutes per session.
“We want to limit muscle loss to no more than 20% of total weight lost,” she explains. That’s why strength training is a foundational part of her guidance, even for beginners.
Start Slow — Especially With GI Side Effects
Because GLP-1s often cause nausea, bloating and reduced appetite, Dbouk recommends avoiding intense cardio in the early weeks. Hydration is also crucial, since patients may struggle to drink enough water in one sitting.
Her tip: “Drink small amounts frequently throughout the day, and avoid getting behind on hydration.”
Once side effects improve, cardio can be added gradually and tailored to the patient’s comfort level and goals.

Whether it’s walking with friends, dancing, swimming or yoga — if it feels like punishment, it’s not sustainable.

While exercise alone won’t drive most of the weight loss on GLP-1s, it’s a powerful tool for maintenance, improved heart health, better cognition, stress relief and long-term wellness.
